The check-in for local teams is Thursday April 25 between 4:30-8:30pm
Location: Courtyard by Marriott. 2501 Bass Pro dr Council Bluffs.
Only one representative from each team needs to be present for check in. No players need to attend. Out of town teams may check in at Tournament Headquarters at the Council Bluffs Recreation Complex on Friday evening between 4:00 pm and 8:30 pm, or on Saturday at least one hour prior to their first game.
ELECTRONIC COPIES OF FORMS / CARDS WILL NOT BE ACCEPTED. Items in italics will be kept for the tournament files.
Official team roster (approved by your association). PLEASE VERIFY BEFORE CHECK IN that number of players, including guest players, does not exceed maximum for the age group AND no player is older than the birth year requirements.
Guest player form (as required by your association) (maximum 5 guest players per team)
Permission to travel form (unless team is from Iowa or a member of US Club Soccer or USSSA)
Player cards for all players and coaches, including guest players (hole punched, order by last name)
Medical releases for all players, including guest players (notarization not required)
